IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

In compliance with the Affordable Care Act (ACA), the City of Monroe is required to provide Form 1095-C, which outlines your health insurance coverage for the previous year.

2025 Form 1095-C is now available upon request.

If you would like to retrieve a copy of your Form 1095-C, you may access it through Employee Self-Service at https://employee.monroenc.org/ess.

To access, select Pay/Tax Information from the menu, then 1095-C. By selecting the “View 1095-C image” link on the far right corner, you can download a printable document.  

Or, if you prefer a printed copy mailed to you, please contact the Human Resources Department via email at hr@monroenc.org.

Requests can be made at any time. The form will be provided to you within 30 days of your request.
